Getting Back into a work out routine

I mentioned last week that I have not over the past few months I was not working out. In anticipation for working out again I bought a workout diary a couple months ago. There is a section in the diary for completing a Physical Measurements and Capabilities Assessment and Cardiovascular Fitness Test. This past week I recorded my Physical Measurements and Capabilities Assessment. This weekend I will complete the Cardiovascular Fitness Test.
The diary allows you to record your current and target measurements. I'm not posting my current and target physical measurements at this time, but I will provide the other items I recorded.

Below are the capabilities I recorded in case you are interested in recoding them for yourself. In the diary I recorded my current capabilities and my target capabilities.
Time how long it takes to run 1mile/2km. 30 minutes
Count how many push-ups you can do before you have to stop. 1
Count how many sit-ups you can do before you have to stop. 1
Count how many squats you can do before you have to stop. 30
Sit with legs out straight. Place a ruler on the floor with the tip between your feet. Record how many inches/cm you stretch up to or beyond your feet. 4.5 inches beyond my feet
Time you can balance on one foot.
Right foot: 1 minute
Left foot: 42 seconds
